I fully understand and accept that any and all retreat activities are undertaken at my own risk.

I fully understand and accept that neither Hope Valley Church, nor its Council, Employees, or Volunteers accept responsibility for any loss, injury or damage that my person or property may sustain whilst attending the retreat, including transport to and from the event.

I hereby consent to medical, hospital, and/or rescue procedures being employed in my best interests if required and understand that my emergency contact shall be notified as soon as possible.

I understand that it is my responsibility to provide and self-administer any required prescription medication for the duration of the retreat in accordance with a current treatment plan authorised by a medical professional. 

I understand and accept that this is a drug and alcohol free event. If such prohibited items are found on my person or belongings I understand and accept that I will be required to depart the event. 

I agree and authorise that photos, statements, audio visual recordings, video and sound bites taken, recorded and collected during the retreat may be used free of charge and at the discretion of Hope Valley Church as part of their marketing and communication.
